Thank you for your interest in our housing program. Please follow the steps below to ensure that your pre-application is processed in a timely manner:
- You must be 19 years or older to complete an housing application
- Please complete the entire form.
- Everyone 18 and older needs to sign and date below

Once your pre-application has been received, you will be placed on our pre-application waiting list. This does not mean you are approved. When a unit comes available that meets your housing needs, a member of our staff will send out an application interview letter with a scheduled appointment date and time. If you miss your scheduled appointment without calling to reschedule, you will be removed from our wait list. (You will have to begin the pre-application process again.) If you do not have all the documents that apply to you, you will be asked to reschedule.

* If you currently owe an outstanding utility bill, this must be paid prior to applying.
* If you currently owe a previous rental property, this must be paid prior to applying.
*We do not have a time frame when a unit will be available.
Thank you for your interest in our program. Below is a list of documents that will be required once your name comes to the top of the waiting list. When your application reaches the top of the waiting list, you will contacted and you will be required to provide the documents below in order to qualify for admission. Please DO NOT bring these documents until requested. You may complete the pre-application online, email it back (please no screenshots), mail or turn it into our offices when open, or drop in our drop box (904 Hickory Street Tallassee, AL 36078), or fax (334) 283-2982.
Items REQUIRED:
- Photo IDs for each adult
- Birth Certificates and Social Security cards (Originals) for each household member, regardless of age
- Proof of all income
- Wages (minimum three (3) paystubs) or
- Contributions (if you receive contributions from family/friends, must have notarized letter stating the amount received each month)
- Child support from absent parent (current statement or notarized letter with amount received)
- Proof of all government benefits received (benefit letters from social security, job and family services, unemployment, food stamps, TANF child support, etc.)
- Proof of all bank accounts (most recent six bank statements for checking, most recent bank statement for savings)
- Proof of cash value of all other assets (statements for 401K, whole life insurance, stocks, bonds, annuities, etc.)
- If the head of household, co-head or spouse are 62 or older and/or disabled, proof of all medical expenses within the last year (doctors, hospitals, prescriptions, health insurance premiums, etc.)
- Current lease or if you're living with family, a statement from the head of household of where you are living (signed, dated and a good phone number)
- If you pay child-care for work or school, proof of monthly payments
- Last year's Tax Return
